Transitional Housing & Reentry Program

Chains to Change–Legacy Corp’s Transitional Housing & Reentry Program is designed for adults recently released from incarceration and veterans who are committed to recovery, accountability, and rebuilding stability. This program provides a structured environment where participants can establish routines, meet expectations, and prepare for long-term independence.

Participants progress through program phases based on consistency, responsibility, and readiness—not time alone.

Phase 1: Stabilization (shared rooms only)

Phase 1 supports individuals as they adjust to reentry and re-establish daily routines. This phase emphasizes accountability, structure, and support while participants build consistency and confidence.

Included Support:

Regular check-ins focused on accountability and progress
Structured household responsibilities
Routine-building and daily structure
Employment navigation and community resource connections, as needed
Supportive peer environment

Progression Criteria:
To advance to Phase 2, participants must demonstrate consistent engagement and accountability, including:

No significant rule violations
Reliable participation in check-ins
Maintenance of routines, cleanliness, and responsibilities

Completion of Phase 1 prepares participants for increased independence and responsibility.

Phase 2: Progression Toward Independence

Phase 2 is designed for individuals who have demonstrated readiness for the next stage of independence. Participants experience increased autonomy while maintaining accountability through ongoing check-ins and expectations.

This phase focuses on:

Sustaining stability
Strengthening independence
Preparing for long-term housing and self-sufficiency

🛠️ Life Skills Workshops

We’re developing a series of practical workshops to equip participants with essential life skills for independent living. These workshops will cover areas like budgeting, meal planning, time management, healthy routines, and communication — all designed to support personal growth and everyday success.

🤝 Peer Mentorship

Our upcoming peer mentorship program will connect clients with mentors who have lived experience navigating reentry and rebuilding their lives. This initiative will create a supportive, understanding environment where participants feel seen, guided, and encouraged through one-on-one connection.

💼 Employment Navigation

We help clients gain meaningful employment through resume assistance, job search coaching, and partnerships with local employers. The goal is to remove barriers and provide personalized support that helps individuals find stable, dignified work opportunities.
